Angiolipoma of the chest wall: a case report

BackgroundAngiolipoma is a rare histological variant of lipoma. Angiolipoma commonly occurs in the subcutaneous tissues of the extremity and trunk. There are few reports of angiolipoma occurring in the chest wall.Case presentationA 78-year-old woman was referred to our hospital for evaluation of angina pectoris. Coronary computed tomography (CT) showed a soft tissue nodule in the left chest wall by chance. Enhanced chest CT showed a heterogeneous enhanced nodule in the left chest wall. On magnetic resonance imaging (MRI), the lesion showed low signal intensity on T1-weighted images, heterogeneous high signal intensity on T2-weighted images and high signal intensity on fat-suppressed T2-weighted images. The lesion showed heterogeneous enhanced effect on gadolinium-based contrast agent. These radiological findings suggested neurogenic tumor with abundant blood flow or hemangioma. Video-assisted thoracic surgery (VATS) was performed for both diagnostic and therapeutic purposes. Histopathological examination of the tumor showed mature adipose tissue and capillary hyperplasia containing fibrin thrombi. These appearances were consistent with angiolipoma. She had an uneventful recovery and did not show recurrence until 8 months post-surgery.ConclusionsAngiolipoma of the chest wall is extremely rare. Preoperative diagnosis is very difficult because the imaging findings of angiolipoma vary depending on the amount of vascular component and fat component, so surgical resection is suggested to be both diagnostic and therapeutic.

SPINDLE CELL LIPOMA OF THE ORAL CAVITY: CASE REPORT OF AN UNCOMMON AND DISTINCT VARIANT OF LIPOMA

Spindle cell lipoma (SCL), an uncommon histologic variant of lipoma, is a benign tumor that often occurs in the posterior neck or shoulder with rare occurrence in the oral cavity. It comprises about 1.6% of intraoral lipomatous tumors. The aim of the case report is to present the histopathologic features of a SCL. A 60-year-old woman with noncontributory medical history was examined for an asymptomatic nodule in the left buccal mucosa. The intraoral examination showed a 7-mm sessile and well-circumscribed nodule with firm consistency and smooth surface covered by slightly whitish mucosa. Excisional biopsy was carried out based on a clinical diagnosis of inflammatory fibrous hyperplasia. The histopathology demonstrated mature adipose tissue, spindle cells (CD34+), and many mast cells in a connective tissue stroma. The diagnosis of SCL was hence established. The patient is being followed up.

OSTEOLIPOMA IN BUCCAL MUCOSA: A RARE VARIANT OF LIPOMA

Osteolipoma is an extremely rare histologic variant of lipoma that contains mature lamellar bone. We report a case of a 27-year-old man with history of a small, rounded lesion in the right cheek that started in childhood, without symptomatology, but with continuous growing. On the examination, facial asymmetry was observed, without lymph node enlargement. A firm nodule of about 2.5 cm was seen involving his right buccal mucosa. A panoramic radiograph showed radiopaque irregular shapes in the region of the first upper and lower right molars. The lesion was excised, and pathologic examination of the specimen showed fragments of trabecular bone between adipocytes. Definitive pathologic diagnosis was osteolipoma. There was no recurrence at 7 months’ follow-up. Although osteolipoma is very rare, it is important to keep it in the differential diagnosis of nodules with a firm consistency in the oral mucosa and when noting a radiopaque image on panoramic exam.

Lipoma is the most common benign tumor of the oral cavity. However, it is extremely rare to encounter chondrolipoma, a histological variant of lipoma that includes mature cartilage tissue in the lipoma. In this article, we describe a case of chondrolipoma arising in the tongue. A 15-year-old girl visited our clinic because of an asymptomatic mass of the tongue. On examination a relatively firm, nonulcerated, yellowish mass 8 mm in diameter was found in the right lateral border of the tongue. The mass was excised with the patient under local anesthesia. Histopathologically, the specimen was composed of mature adipose tissue and cartilaginous tissue surrounded by abundant myxoid and fibrous tissue. There was no cell atypia or mitosis in the specimen. The histopathological diagnosis was chondrolipoma. There was no recurrence during 9 months of follow-up.

Giant angiolipoma of the arm in an elderly patient

Introduction Angiolipoma is a histological variant of lipoma and is the most common neoplasm in the trunk and extremities of young adults. It is extremely rare in elderly people, and its size is ≤4cm. Few data are available for large angiolipomas. Case History An 86-year-old patient was admitted to our surgical department due to a large mass on his left arm, which was resected. The specimen measured 19.5 × 15 × 10.5cm. Histopathological examination revealed a benign non-infiltrating angiolipoma. This is the first report of a giant angiolipoma of the arm reported in an octogenarian patient. Conclusions Giant lipomas of the upper extremities are extremely rare. Resection is associated with cure in most patients, but regular follow-up should be considered.

Fibrolipoma of the Buccal Mucosa: A Case Report and Review of the Literature.

Lipomas are common benign soft tissue neoplasms derived from mature adipose tissue. However, they rarely arise in the oral cavity. Fibrolipoma is a histological variant of lipoma that mainly affects the buccal mucosa and causes functional and cosmetic issues. This article describes the case of a 71-year-old male with a fibrolipoma of the left buccal mucosa and a review of previous articles about fibrolipoma.

Electrosurgical Management of a Rare Case of Intraoral Fibrolipoma of Buccal Mucosa: A Clinical and Histopathologic Case Report

Introduction: Fibrolipoma, a histologic variant of lipoma, mostly affects the buccal mucosa and causes functional and cosmetic disabilities. Oral lipomas are benign and rarely seen in the oral cavity. Fibrolipoma of the oral cavity has also been infrequently reported. This report presents a rare case of intraoral fibrolipoma detected on the right buccal mucosa. The lesion was removed using electrosurgery (ES).Case Presentation: A 40‐year‐old female patient presented with a chief complaint of a painless growth on the right buccal mucosa causing irritation when opening and closing the mouth. Provisionally diagnosed as irritational fibroma, the growth was excised using ES under local anesthesia. No sutures were placed, and the bleeding was controlled in the intermittent coagulation mode. The histopathologic report revealed the lesion to be a fibrolipoma. Healing was uneventful, and no recurrence was observed for a period of 1 year.Conclusions: Compared with scalpel surgery, ES requires more expertise, but the advantages counter the technical complexities. Minimal bleeding and postoperative discomfort and good patient compliance were observed in this case. Therefore, ES may be considered as a patient‐ and clinician‐friendly tool, although a long‐term comparative assessment with scalpel surgery is recommended to establish its effectiveness.

Pure Intramuscular Osteolipoma

Ossified lipoma or osteolipoma are rarely reported. It is defined as a histologic variant of lipoma that has undergone osseous metaplasia. Osteolipoma presents with a dominant osseous component within a lipoma. We report a case of a histologically confirmed osteolipoma on the nuchal ligament independent of bone. The patient was a 51-year-old female who presented with a 5-year history of a painless, progressively enlarging mass on the posterior neck. Computed tomography and magnetic resonance imaging showed a circumscribed mass compatible with fat between the C2 and C6 spinous processes with a large calcified irregular component. The mass with dual components was totally removed under general anesthesia and no recurrence was observed after 6 months of follow-up. We also reviewed the clinicopathologic features of previously reported osteolipomas in the literature and suggest that although osteolipoma is a rare variant of lipoma, it should be considered in the differential diagnosis when a lipoma of the posterior neck mixed with a bony component is encountered.

The lipoma is a very common benign tumor of adipose tissue, but its presence in the oral and oropharyngeal region is relatively uncommon. Fibrolipoma, a histological variant of lipoma, mostly affect the buccal mucosa and causes functional and cosmetic disabilities. Hence, accurate histopathological examination of lipomas is important for a correct treatment plan. This article describes a case of 10 year old girl with fibrolipoma of the buccal mucosa with a relevant review of tumors.
